FOREVER MOMMY PHOTO CONTEST OFFICIAL CONTEST RULES


ELIGIBILITY:
The Forever Mommy photo contest is open to legal residents of the Philippines and to Filipinos outside the Philippines, who are (a) 18 years of age or older and (b) the parent or legal guardian of a child that is between 0 and 5 years of age at the time of entering the Contest.

HOW TO ENTER:
1. Provide the following requested information:
a. Your complete name
b. Address
c. Contact Number
d. Email Address
e. Complete Name of Child
f. Age and Gender of the Child
g. Your relationship to the child
h. Birthdate of Child

2. Submit 1 photograph of your child. The Photo must be your original photograph and not a professional photograph. It must not be edited or filtered.

3. You must also pay the PhP150 entry fee thru BDO Bank Deposit, BDO/BPI Online Transfer, Paypal, Cebuana, LBC, Palawan, Western Union, M Lhuillier, Smart Padala and Paypal. We accept GCash as well but there will be additional PhP10 charge. After payment, you need to send a copy of proof of payment to us. Registration fee is non-refundable. Those that will send fake receipts or reference numbers (Smart Padala) will be posted in the page as scammers.

SELECTION OF WINNER:
The criteria for determining the winning photograph will be thru the following:

30% Facebook Likes and Reactions
10% Instagram Likes

The use of robotic or automatic devices for voting is prohibited and Forever Mommy reserves the right to nullify any and all such votes and to disqualify from voting the responsible individual.

60% Judging (Appearance, Charisma, Expression, Image Concept/setting, That “special something”) Additional points will also be given if candidates will purchase products in www.forevermommy.com.

Winners will be determined within 5 days after the end of the Contest; however, Forever Mommy reserves the right to take more or less time to determine the winner as it deems necessary in its sole discretion.

LIMITED TIME ONLY:
The Contest period begins April 1, 2018 and ends April 15, 2018.

PRIZE DESCRIPTION:
The grand winner will receive a total of PhP10,000 of which PhP5,000 is cash and PhP5,000 worth of Forever Mommy Products and will be featured in Forever Mommy 2019 Calendar. Three (3) runner ups will receive PhP1,000 worth of Forever Mommy products. Winning photos may appear on the home page of the Forever Mommy Website, where potentially thousands of viewers may see the photograph. Entrants waive any and all rights to photographs and give Forever Mommy the right to publish same in any medium without compensation.

Part of the proceeds will be donated to unfortunate children and mothers in the Philippines.
