Terms and Conditions

     1. Customer can participate in the Shell feedback promo with every
       purchase (no minimum amount) of any Shell Quality Fuels, Shell
       Select and Shell Helix Service Center.

     2. Customer who completes the Shell feedback form either thru web,
       may get the chance to win free fuel worth P 5,000.00.

     3. Customers can complete the feedback form by visiting
       http://www.shell.com.ph/customerfeedback

     4. Thirty (30) winners will be determined through electronic raffle
       draw every month. Ten (10) from Metro Manila and Five(5) winners
       each from North Luzon, South Luzon, Visayas and Mindanao. Raffle
       Draws will held at OksPinoy Inc. office, Crown Equities Inc, 2/F
       Room Crown Center, 158 Jupiter cor N. Garcia, Makati City on the
       following dates:

     * c. April 6, 2018
     * d. May 4, 2018
     * e. June 8, 2018
     * f. July 6, 2018
     * g. August 7, 2018
     * h. September 6, 2018

     5. Cut off for submission of entries per draw date are as follows.
     * a. February 9, 2018 raffle draw: January 31, 2018 11:59PM

     * c. April 6, 2018 raffle draw: March 31, 2018, 11:59PM

     6. Non-winning entries from previous draws are entitled join the
   succeeding draws.

     7. Customer must keep the original receipt as he/she will be asked to
   enter the OR number and Shell site’s SHIP-TO number (site ID Number) in
   the web survey. Only one (1) OR number will be accepted.

     8. Customer must complete the web survey and he/she will be given a
   confirmation code which will serve as his/her raffle entry.

     9. A customer can only win once.

     10. Winners will be notified by a Shell Representative through call
   and registered email. Winners can redeem their prize sixty days (60)
                        Shell Philippines Customer Satisfaction Survey (p4 of 5)
   upon receiving of registered email. Winners must present the registered
   email, preferred station to claim their prize and at least 2 valid IDs
   (e.g. license, SSS ID, passport, company ID, etc. Once verified, Shell
   Representative will confirm to winners that they can already claim
   their prize on their preferred Shell station.

     11. Fleet, PO, and bulk transactions are entitled to join the promo.

     12. Employees of Shell Companies in the Philippines (SCiP), dealers
   and retailers, its advertising, BTL, and PR, and media agencies and
   their relatives up to 2nd degree of affinity or consanguinity are
   disqualified from joining the promo.

     13. Promo is only valid at participating stations nationwide.
